I have a variant of this that I use with a little more space for post-session commentary. 

 

I like to laser-print them on medium-weight card-stock.  They stand up better to conditions at the track (wind, mist, rain, etc).  I'm also considering printing them double-sided this year with a track-map on the back.  20 per weekend covers it (with a few spares).

Thank you for the template, should be of great value for the upcoming season. I like the idea of using a thicker card stock as well. I would go a step further and laminate one and use a dry erase marker for a master setup sheet that stays on-top of your toolbox.

I keep each sheet as a record.  It gives a concise view of the setup, the track conditions, the driver reactions, and the changes made to adjust.  That info along with the follow-up sheet (how the car responded to the changes)...goes a long way towards learning how to adjust the car....well, that and a brain smart enough to comprehend what the data tells it.
